Valley View ISD parents and guardians are reminded that new parent consent forms are required for students to receive counseling and certain health-related services at school in accordance with Senate Bill 12 (SB 12).
Parents must complete both required forms for each child. If you have multiple children enrolled, the forms must be submitted separately for each student.

Without signed consent, staff may only provide limited services, except in medical emergencies or situations involving immediate danger.
