Valley View Family Medical Clinic will be available on Tuesday, May 7th, during HS boys' and JH girls' athletics and on Wednesday, May 8th, during HS girls' athletics, 6th-grade PE, and JH boys' athletics. They will conduct physicals for students intending to participate in athletics, marching band, or cheer for the upcoming school year 2024-25.

The cost for the physical examination is $20, payable in cash or by check. Before the physical examination, the medical history section on the front side of the form must be completed.
