Notice to Vendors
In accordance with Texas Local Government Code Chapter 176, vendors who contract or seek to contract with Valley View Independent School District for the sale or purchase of property, goods, or services may be required to file a Conflict of Interest Questionnaire (Form CIQ).
It is the responsibility of each vendor to determine whether a filing is required under Chapter 176 and to comply with all applicable disclosure requirements.
Conflict of Interest Questionnaire (Form CIQ)
A completed Conflict of Interest Questionnaire (Form CIQ) must be filed no later than the seventh business day after the date the vendor:
- Begins contract discussions or negotiations with Valley View ISD;
- Submits a bid, proposal, or other response to a solicitation;
- Submits correspondence or another written communication related to a potential contract; or
- Otherwise becomes subject to the filing requirements of Chapter 176.
Vendors must also file an updated questionnaire by September 1 of any year in which a reportable activity is pending, and no later than the seventh business day after any event that causes previously reported information to become incomplete or inaccurate, as required by Texas Local Government Code §176.006.
Note: A vendor is not required to file an updated questionnaire if one was filed on or after June 1 but before September 1 of the same year.
